Certificates of hospitalization or of outpatient follow-up are supplied by the "Certificates Office" of the Hospital on request.
Office is open on weekdays from 09:00 to 13:00.
The application can also be submitted via the Citizen Service Centers (Kentra Eksypiretisis Politon - KEP) and sent by fax to the Secretariat at 2107228297 or by post to the postal address of the Eginition Hospital with a note to the Secretariat.
The certificate is received after the patient has been informed by telephone:
• By the patient himself, on demonstration of a Police ID .
• From another person, ONLY with an authorization approved by the Police Authority or KEP.
In case of death, legal heirs must provide:
a. Certificate of close relatives
b. Death Registry
c. Certificate of non-publication of a will and, in the case of a published will, certificate of non-publication of another will & copy of the will.
d. In case of non-heirs who have a legitimate interest, an order from a Public Prosecutor must be submitted.
